MDA GAP/GHP Request Form

MDA has developed a State GAP/GHP program that addresses basic food safety requirements, intended for farmers who haven’t been audited before. Producers who participate in MDA's GAP/GHP program will submit the request form in order to schedule an inspection on their farm. View Details

MDA SLF Permit Form

This form captures the information for people who have a Spotted Lanternfly (SLF) Permit. Due to the quarantine to prevent the spread of this invasive insect, all businesses moving regulated articles within or through an area known to be infested with SLF must obtain a permit. This information is public and should be viewable (but not editable) by the public, but should be editable on the back end to add this information. MEQAP Processor Participation Agreement
