USDA GAP/GHP Audit Request Form
General Info - Statewide Registration
MDA has a cooperative agreement with USDA to conduct audits of fruits and vegetables in compliance with USDA’s Good Agricultural Practices and Good Handling Practices standards and the Pre Harvest and Post Harvest Harmonized standards. Applicants who wish to be audited to these standards must submit a request on USDA’s forms “GAP/GHP audit request form”.
Auditee will indicate on the request form when they would like to have the audit conducted. Once the audit is completed the average wait time from USDA is 2 months to receive the certificate.
Time to complete audit varies for each request. Cost is $150/hour of time spent on the audit, including travel time.
