  1. Please use this form ONLY with a tick specimen included. We cannot make identifications without the tick.

  2. Instructions for preserving and mailing the tick with the form are at top of the PDF document.

  3. There are two parts to the form, just fill out the top of the document. The state entomologist fills out the bottom half. Once we receive the tick specimen and form together and we’ve made an identification, then Maryland will mail a letter with that information to you.
