Obtaining the Registration

Ready to get your registration? - Steps, Requirements, and Documents

  1. Provide business/public agency name and address.

  2. Provide business license/ public agency permit number.

  3. Provide telephone number.

    1. List terminated employee name, social security number, indicate if employee is REGISTERED or PROVIDE CERTIFICATION number, date of termination, indicate if ID is returned.

  5. If ID card is returned, please mail it to Pesticide Regulation.

  6. Provide name of Authorized Individual.

  7. Authorized Individual signature and date required.
