Application for a license to operate a nursery/plant dealer or plant broker business for new business

The approval time is based on thorough completion of all documents and payments received.

Obtaining the License

Ready to get your license? - Steps, Requirements, and Documents

  1. List the owner or proprietor first and last name.

  2. List the company/firm name as it will appear on the license/certificate.

  3. Circle the type of business, types of products and sales structure of your business.

  4. List your mailing address.

  5. List, if any, additional sales locations (Provide complete address, manager/contact name, phone number and information from questions 6-12. Use a separate sheet of paper if necessary. (Each sales location is required to pay a separate fee).

  6. Principal place of business, if different from item 5.

  7. Contact Information of headquarters: Phone, Fax, Cell phone, Email

  8. If a partnership or association, list the name and address of each partner or association officer.

  9. If corporation, list date incorporated, State, FID #, Address of principal office, Name & address of resident agent.

  10. Doing business as statement that is notarized.

  11. List locations of growing areas if different from or in addition to principal place of business (not sales areas, does not require license for growing areas).

  12. Combined acres for stock production, sales area, and landscape holding area.

  13. Greenhouse space listed in sq.ft..

  14. Check either yes or no, if yes list the policy # or binder # and expiration date

  15. Information for Landscape Contractors.

  16. List the businesses that you receive your plant stock from including company name, address, city, state, zip code, and phone number (use a separate sheet of paper if necessary).

  17. License and inspection fee schedule (Make checks or money orders payable to the Maryland Department of Agriculture).

  18. Sign, list your title and date.
