General Info - Statewide Registration
The purpose of the notification form is to identify and to evaluate underground tanks that store, or have stored, regulated hazardous substances, including all oil products in the state of Maryland. It is expected that the information provided will be based on reasonably available records, or, in the absence of such records, knowledge, belief, or recollection.

The notification form (MDE/WAS/PER.012) is completed expeditiously by the owner, operator, or person responsible for all tanks currently and previously used to store regulated substances at each location owned. The completed notification form should be mailed to:
