  1. Proof of Board approved training for all staff members conducting euthanasia

  2. CDS Registration Certificate from the Maryland Department of Health

  5. Print and fill out a Request for Animal Control Facility License Application. Mail the Request and the licensing fee to the address provided on the Request.

  1. Certificates of euthanasia training for staff members must be submitted every year.

  2. To renew an existing license, contact the State Board of Veterinary Medical Examiners' Office at 410-841-5862.
