To operate legally within Maryland’s tobacco Industry, businesses must obtain the appropriate licenses and permits.

This form can be used to apply for different types of Tobacco Licenses as below:

Electronic smoking devices (esd) license application

  1. Manufacturer # VM- $25.00
  2. Wholesaler Distributor # VW- $150.00
  3. Wholesaler Importer # VI- $150.00

Cigarette and other tobacco products (otp) licenses application

  • Cigarette Licenses

  3. Vending Machine Operator TV $500
  4. Manufacturer TM $25
  5. Storage Warehouse TP $25
  • OTP Licenses
