Free VFD Form and Prescription Upload


Maryland Senate Bill 471, passed in April 2019, requires licensed veterinarians to submit the following to the Maryland Department of Agriculture (MDA), beginning 2020:

  1. a copy of all records (prescriptions) prescribing medically important antimicrobial drugs in water to cattle, swine or poultry
    (and/or)
  2. a copy of the Veterinary Feed Directive (VFD), for each medically important antimicrobial drug, administered in feed or water to cattle, swine, or poultry for each year beginning 2020.

Veterinarians must submit the prescription or VFD records noted above to the Department quarterly, beginning June 30, 2020. Records may be submitted by:

  1. scanning records and uploading them through this portal or

  2. by inputting data from your VFDs directly into this portal

You can also use this digital form to creating VFDs, at which point the data is submitted to MDA automatically for your convenience. There is no fee for record submission.

Going forward, quarterly submissions are due on the following dates this year:

  1. June 30th
  2. July 15th
  3. September 15th
  4. January 15th

Don’t be concerned about the terminology of “apply”. It is a generic button used throughout the portal to give access to all kinds of forms.

Review the steps - read carefully

  1. Bookmark this page for your convenience.

    You can usually do this by selecting the star icon () at the end of the address/search bar in your browser.

  2. Log in, if you haven't already.

    Enter your OneStop username and password. If you haven’t created one, you’ll have to do that by selecting Register.

  3. Select Apply Online.

    Don’t be concerned about the terminology of “apply”. It is a generic button used throughout the portal to give access to all kinds of forms.

    If you try to select Apply Online without logging in first, you will be asked to log in. You may have to create a registration to do so.

  4. Decide whether you want to scan and upload, or manually key-in data.

    At the prompt Would you prefer to upload a complete, paper VFD or input your VFD information below? select which of the two methods you want to use for reporting your VFD.

    Upload VFD or Prescription

    If you already have a digital version, you can upload it. If you have a printed VFD or RX, you can scan it as a PDF using a scanner or a mobile phone app. Then select this option and select Choose Upload for a prompt to upload your VFD file.

    Manually Key-In VFD Information

    If you haven’t already written the RX or VFD yet, select this option to input the VFD or RX data. At the end of entering the relevant information, you can save the completed VFD as a PDF for sending to appropriate recipients.

    To view and print your VFD to a PDF file that you can distribute, select the Print option, located near the top of the form.

  5. To View Or Print A VFD or Prescription That You Have Submitted
    1. Make sure you are logged into OneStop.
    2. Select My Dashboard.
    3. Under Your Recent Applications, again bearing in mind that “application” is a generic term used by the portal, select the link labeled Free VFD Form and Prescription Upload that corresponds to the VFD that you want to view.

    You will see the completed VFD or prescription.
    View the completed VFD or prescription. If you want, you can print the VFD by selecting Print, usually located near the top of the form.
