Maryland Department of Health-issued certification for experienced Community Health Workers (CHW) who have met the requirements to be certified. Certification for CHWs is valid for 2 years from the date of issuance.  


IMPORTANT UPDATE:  THE DEADLINE FOR APPLICATIONS FOR COMMUNITY HEALTH WORKER (CHW) CERTIFICATION BASED ON EXPERIENCE IS JUNE 30, 2021 !!

Applications for CHW certification based on experience will only be accepted through June 30, 2021. Applications in "Draft" mode in Maryland OneStop will not be considered unless they are submitted by June 30, 2021. Once this pathway closes, the only pathway to CHW certification will be based on the completion of an accredited CHW certification training program!

  1. CHW applicants must submit their applications for certification based on experience by June 30, 2021.

  2. *Important* Visit the Maryland Department of Health Community Health Worker Certification website at https://pophealth.health.maryland.gov/Community-Health-Workers/Pages/Certification-Program.aspx, prior to beginning your application, for full application instructions, information, and the required downloadable letter of validation form.
  3. Complete online application and provide required documentation.
  4. Submit application. For the best online application experience, please use Mozilla Firefox, Google Chrome or Safari as your web browser (you may experience issues using Internet Explorer).
