Community Health Worker Certification Application


Community Health Worker (CHW) Certification

 

Individuals may apply at any time for community health worker certification.


Certification for CHWs is valid for 2 years from the date of issuance. CHWs must renew their CHW certificate every two years by submitting a CHW Certificate Renewal Application. An Individual may apply for the reinstatement of an expired CHW certificate by submitting a complete CHW Renewal application up to two (2) years after the certificate expiration date.

Applicants should visit the Maryland Department of Health Community Health Worker Certification website at https://pophealth.health.maryland.gov/Community-Health-Workers/Pages/Certification-based-on-the-completion-of-an-Accredited-CHW-Training-Program.aspx, prior to beginning their application, for full application instructions and information.

  2. Applicants may apply at any time.

  3. Complete online application and provide required documentation. Once submitted, please do not start or submit another application.
